Certificate of Insurance

Other businesses may request that we provide them with a "Certificate of Insurance" as proof of the University's insurance coverage.

CU may require proof of insurance from companies, organizations, or other groups for use of campus facilities for events, and the like. 
This following pdf file is a sample Certificate of Liability Insurance with the required limits and coverages shown.  Special attention should be paid to completing the form as shown in the sample.  Incomplete or incorrect certificates will result in a delay of approval.  The University reserves the right to negotiate alternative coverages and limits based on the type of event.

To request a certificate of insurance, please send your written contract or agreement (or complete details of your event), the insurance requirements, the name, address, phone number, and contact person of the company requesting the certificate, and your name and department information to:
