Certificate of Insurance
1) Third Party Request for a CU Certificate of Insurance
A) When CU is holding an event or activity at an off-campus location, the off-campus/non-CU property owner may ask for proof of CU’s insurance.
B) When CU contracts with a third party (non-CU) company, that company may ask for proof of CU’s insurance.
To request the certificate, send University Risk Management the following information:
- Brief description of contract/event
- Beginning and end dates
- Name and address of Certificate Holder (Non-CU requester)
- e-mail of Certificate Holder, if known.
- CU contact name, phone number, and e-mail or Fax No.
- Insurance limits, if specified (if not specified, standard insurance limits will be shown on Certificate.)
Please note that the University of Colorado is unable to add an Additional Insured endorsement for any reason. If that is requested, a form letter will be sent with the Certificate. Click Here to download the form letter.

Expected turnaround time is no more than two business days, and usually within one business day.
2) CU Request for Proof of Insurance from Third Parties
For certain On-Campus Events and agreements/contracts, CU-Boulder requires proof of insurance.
The following pdf file is a sample Certificate of Liability Insurance to help guide third-parties to properly provide proof of insurance.
Incomplete or incorrect certificates will result in delays.
The University reserves the right to negotiate alternative coverages and limits based on the type of event or contract.
For special risks, including hazardous materials, construction, and RFP’s, additional insurance requirements apply. Contact your URM Campus Operations office for further information.
