How to apply for Camp Insurance Coverage*
 
Campus Department Responsibility:

  • Register on line no later than 15 days prior to the camp: Camp Insurance Departmental Information Form.
    A preliminary list of participants will be sent to the URM Camp Coordinator with all pertinent information regarding each participant including type of health insurance coverage.
  • Schedule a risk assessment meeting with your campus Risk Director/Manager:
    • UCD: Mary Beacom 303-315-2731

  • All facilitators, staff, volunteers, etc., who are involved in the camp should be included on the preliminary list.
  • Once the camp is completed, send an updated list of participants who attended.
URM Camp Coordinator Responsibility:

The URM Camp Coordinator will assign a Camp number once the Departmental Information web registration form has been received.

  • An E-mail to the department acknowledging receipt of the application with an assigned Camp Number.
  • Upon receipt of the final camp participants list, the premium will be charged to the campus department's speed type account number.
  • Premium calculation: number of participants x number of days x rate per day.
  • Notify insurance carrier for coverage
