FILING A CAMP CLAIM FOR CAMP INSURANCE COVERAGE
Campus Departments are responsible for the following:
- A Claim Report must be completed and sent to the University Risk Management Office as soon as possible after the incident at 303-860-5680 (fax) or sent to:

- The Claim Report must be signed by the camp director, chaperone, group leader, or department designee (unrelated to the claimant).
- If the Campus Department has the medical diagnosis by the attending physician and/or any itemized statements for services rendered by the doctor or hospital, please attach to the Claim Report.

University Risk Management is responsible for the following:
- URM Camp Coordinator will send all documents to the insurance carrier for processing.
- URM Camp Coordinator will work with the insurance carrier to ensure timely payments are processed to the provider.
- URM Camp Coordinator will act as liaison with the medical providers and campus departments to ensure all claims are processed accurately.
