WORKERS' COMPENSATION CLAIM PROCEDURE
On-the-job needle-stick, exposure, injury or illness

NEEDLE-STICKS OR BODY FLUID EXPOSURES will initially be seen in the Emergency Room of the hospital where the Workers’ Compensation incident occurs.

Exceptions are: University of Colorado Hospital - Go to the Infectious Disease Clinic between 8:00 AM and 4:00 PM and the ER after 4:00 PM.

Denver Health Medical Center (DHMC) - The Occupational Health and Safety Center (corner of 6 th Avenue and Bannock, 4th Floor) is available 7:30 AM - 4:30 PM Monday through Friday with no appointment required.  The Emergency Room at Denver Health is also available.  

The Children’s Hospital (TCH)  - (Building directly West of Hospital Entrance-corner of 19 th & Ogden). Employee Health Services 1056 East 19 th Ave. # B260 is open 7:00 AM – 4:30 PM Monday  through Friday, with no appointment required.  

After the first 48 hours, WC needle-stick or body fluid exposure patients should follow-up at one of the Designated Medical Providers listed below or DHMC Occupational Health, depending on where the exposure occurred.
 
      

OTHER W.C. CLAIMS:   All other non-emergent on the job injuries must be seen at one of the following authorized clinics.


In the event of an on the job injury/exposure that does not require immediate attention – please call the clinic of your choice and schedule an appointment to be seen.  They can usually schedule you the same day. 

If the injury requires immediate attention (but not life threatening), you may go to the clinic of your choice on a “walk-in” basis, no appointment required.

SUBMISSION AND PAYMENT OF YOUR CLAIM  
For Exposure claims, you must complete and file the Denver Needlestick/Bodily Fluid(s) Exposure Report Form within four working days of the injury/exposure. For a copy of this form to complete, contact University Risk Management at 303-315-2730.  After receiving treatment, take the Status Report to your Program Director. For all other injures you must complete and submit the Denver Employee's Injury Report Form  

Remember, University Risk Management, not your health insurance, is responsible for payment of services related to an on-the-job-injury/exposure.  Send bills from authorized medical providers for an on-the-job injury to:
