Compensation Claim Procedures
If the injury is not life
or limb threatening, the employee should first notify their supervisor of the incident and
complete the Employee's Injury Report form.
* Anyone located at a specific campus, including Administration employees, should refer to the procedures for that campus and determine the Designated Medical Provider (DMP) by clicking on the link on the above line.
* Once the Designated Medical Provider (DMP) has been identified, please contact them for an appointment.
If the injury is life or
limb threatening, the employee should be sent directly to the nearest emergency room for
immediate medical attention. An ambulance may be requested only if the injury is life or
limb threatening. Employees at the Boulder campus should be sent to Boulder Community
Hospital. Employees who are not from the Boulder campus should be directed to the nearest
available emergency room. Employees who could not immediately notify their supervisors of
the incident should do so as soon as possible after receiving emergency medical attention.

The supervisor or employee should
then complete the Employee's Injury Report
Form and submit it online or if necessary fax it to University Risk Management at 303-860-5680. Supervisors should make
sure to note on the report any unsafe conditions that may have caused or contributed to
the accident. The original report should then be either mailed to University Risk
Management, 1800 Grant Street, Suite 700, Denver, CO 80203, or send it through campus
mail to 014 UCA, as soon as possible. The supervisor retains a copy of the report
for their records. Please note that an employee, by law, has four (4) working days to
notify the employer of the incident who, in turn, has ten (10) calendar days to notify
University Risk Management. Please note that these time frames are very important and critical! The state can impose penalties for
non-compliance amounting up to $500/day or one day's compensation for each day a deadline
is missed.

Employees who have first
sought emergency medical treatment prior to formally reporting the incident should then be
referred to the appropriate designated medical provider (DMP) for medical attention. For
Administration employees, Arbor Occupational Medicine is the designated
medical provider, however, if employees are outside of Boulder or if it is after hours, then employees should go to the nearest medical facility. Please note that, by law, the employer has the right to designate the
medical provider their employees will utilize. Any request for a change to another medical
provider should be directed to University Risk Management.
Upon receipt of the
Incident Report, University Risk Management will assign the claim to an adjuster who will
then investigate the incident and begin the process of determining compensability and
managing the claim from its inception to its conclusion. Any questions regarding a
specific claim should be directed to the assigned adjuster.
